Liability Waiver

Below is a copy of the Liability Waiver which is submitted at the time you register for a class. If you have any questions about the content of the waiver please don’t hesitate to contact us.

Conduct

The Palace Dance Studio participants are under supervision whilst in class time, as well as travel to and from the location. We will endeavour to provide a safe environment, however it is required by you, the participant, that you will follow and adhere to any guidelines or directions by your choreographer, teacher, transport driver or any other personnel who assist with the running of this program.

The Palace Dance Studio maintains the right to dismiss any student who breaches any of the stated Terms and Conditions, damages property or causes disruptions to any classes of a severe nature. The decision is at the discretion of the dance Directors.

Medical

The Palace Dance Studio provides qualified and/or experienced dance teachers, who actively promote safe dance practice and aim to provide a safe learning environment to reduce risk of injury. This being the case, it should be realised that injuries do occur from time to time and students must accept this element of risk.

It is the student’s or parents/guardians responsibility to disclose any medication conditions on the registration form. It is the student’s or parents/guardians responsibility to inform their teacher of any injury or medical condition which is being treated or may be of concern prior to the start of the class / workshop

A First Aid Kit is available to any student through their choreographer or centre administration. In the event of an injury or accident it is the student’s responsibility to inform a staff member and an Incident Report must be completed as soon as practical. 

Transport

From time to time The Palace Dance Studio offers transport for students to/from designated pickup locations within the community and/or to the students pre-approved home address. Any changes to the pre-approved arrangements must be made by the parent/guardian to the programme co-ordinator.

Attendance

Regular weekly class attendance is vital for students to develop and keep up with the syllabus for the year. Students must be punctual to ensure they receive a complete warm up. If a student is not able to attend a class, they must notify the Student Services Manager via txt (to 3548 or 0210 56 76 70) email (kathy@theroyalfamily.co.nz) or phone (0210 56 76 70).

Liability

The Palace Dance Studio is not liable for personal injury or any loss or damage of personal property.

Privacy

By attending the programme you are giving permission to The Palace Dance Studio to use any photos / videos taken of during the class / workshops in promotional materials, both print and electronic media.

This enrolment form collects personal information about you. Under the Privacy Act 1993, you are entitled to have access to, and request correction, this information which is held at Studio 23 Ltd premises at 1E Ryan Place, Manukau, Auckland, New Zealand
